When you contact us or request a booking: your name, phone number, email address, the service you are interested in, a preferred date, and anything you write in your message.
When you book online: our booking system, Calendly, collects your name, email address, phone number and appointment details.
When you receive care: the health information we need to assess and treat you, and insurance or claim details (for example extended health benefits, WCB or a motor vehicle accident claim) when you ask us to bill a third party.
Payment information: we keep a payment method on file for visit fees and our cancellation policy. Your card is stored securely by our payment provider. We do not keep your card number in your patient file, and we do not collect card details through this website.
Website usage: technical information such as the pages you visit, your approximate location, your device and browser, and how you arrived at our site, collected with cookies and similar technologies.
